Home health care starts with a physician's order — not a booking form. Here is exactly what happens, and what we handle for you.

Physician's order
Your doctor or hospital discharge planner refers you to us. Before anything else we verify your Medicare coverage — if it does not cover the care, we tell you rather than send a nurse.

In-home assessment
Once coverage is confirmed, a registered nurse visits your home, reviews your needs and reports back to your physician.

Your plan of care
We build a personalized program around the physician's orders and review it with you and your family.

Care begins
Our team starts scheduled visits, and an on-call nurse is reachable after hours, weekends and holidays.

Home health care is not right for every situation. During the in-home assessment a registered nurse will tell you honestly if it is not right for yours.
