Notice of nondiscrimination
We provide care without discrimination, and we provide it in your language.
Draft document — not yet reviewed by counsel
This page is a working draft prepared during the website build. Before launch it must be replaced with the agency's own notice, reviewed by legal counsel and matched to the policies actually in force.
Our commitment
Advanced Home Health Services complies with applicable federal and Illinois civil rights law. We do not exclude people or treat them differently because of race, colour, national origin, age, disability, sex, religion or source of payment.
Language assistance
Our own staff speak English, Polish, Russian and Spanish, and this website is published in all four. If your primary language is another, tell us and we will arrange assistance at no cost to you.
- Care discussed directly in English, Polish, Russian or Spanish
- Free interpretation arranged for other languages
- Written information provided in other formats on request
If you believe we have fallen short
You may file a grievance with our office in person, by phone or by mail. Filing a grievance will never affect the care you receive. You also retain the right to file a civil rights complaint with the relevant federal or state authority.
