Information we hold
To provide care we collect health information: your diagnoses, medications, physician's orders, visit notes, and the insurance details needed to bill for covered services. We collect only what care and billing require.
How we use it
Your health information is used for treatment, payment and health care operations. It is shared outside the agency only where care requires it or the law compels it.

- With your insurer, to obtain payment for covered services
- Where required by law, or with your written authorisation
Your rights
You have the right to see and obtain a copy of your records, to request a correction, to ask who your information has been disclosed to, to request restrictions on its use, and to receive a paper copy of this notice on request.
